Overview

To provide clinical care and leadership for the Neurology Service line. This is provided within an interdisciplinary model of care, with specific attention to patients at high risk of poor outcomes and transitions in care.

Responsibilities
  • Evaluation & Management: Performs complete history & physical and documents in the EMR. Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
  • Evaluation & Management: APP may serve as a first responder for both nursing and house staff in the event of patient status changes, orders and in situations requiring procedural and resuscitative interventions. Evaluates and re-evaluates acute and chronically ill patients, providing necessary changes to their plan of care routinely.
  • Diagnosis: Collaborates with interdisciplinary health care team making the diagnosis of acute and chronic conditions. Manages diagnostic tests through ordering, interpretation, performance and supervision.
  • Diagnosis: Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions.
  • Plan of Care: Formulates plan of care, identifies expected outcomes, and documents accordingly, addressing complex acute and chronic health care needs. Implements and modifies plan of care.
  • Plan of Care: Utilizes evidence-based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum.
  • Documentation: Documents key components of patient's progress via daily progress note, transfer, and discharge summary. Including but not limited to H&P, progress notes, plan of care, problem lists, procedure notes, acute event note and discharge summaries. Documentation is timely, meets acute care compliance standards and accurately reflects patient's acuity.
  • Communication and Collaboration: Maintains ongoing communication and collaboration with interdisciplinary health care team. Works with Stroke Coordinator to reinforce education to medical staff and nursing staff. Serves on stroke committee and attends meetings.
  • Communication and Collaboration: Facilitates and communicates with patient, family, and staff to promote continuity of care across the continuum. Schedules follow-up appointments with Neurologist prior to discharge.
  • Professional Practice: Demonstrates professional practice behaviors including preceptor/mentoring, education and instruction of graduate students. Participates in specialty-specific meetings, engages in process improvement, development and review of current protocols with both Hospitalists and Stroke Coordinator.
  • Professional Practice: Seeks opportunities for active engagement in research and analysis of evidence-based practice. Maintains membership in professional organization and maintains CEU's.
Qualifications
  • Graduate of Nurse Practitioner program; Master’s degree.
  • Current Delaware APRN license or ability to obtain.
  • Current Basic Life Support (BLS) certification.
  • Advanced Cardiac Life Support (ACLS) certification.
