American Health Network, part of Optum, is seeking a primary care advanced practice provider (NP or PA) to join our Carmel, IN practice. You'll collaborate with a multidisciplinary team to deliver high-quality, value-based care.

Qualifications

• Unrestricted licensure in Indiana before starting.
• Board certification or eligibility as Family Nurse Practitioner (ANCC/AANP) or Physician Assistant (NCCPA).
• Active and unrestricted DEA License before starting.
• Active and unrestricted CSR License before starting.

Responsibilities

• Provide comprehensive primary care to a diverse patient panel with support to practice at the top of your license.
• Collaborate with a multidisciplinary team including care coordinators, behavioral health, pharmacy, and other care providers.
• Perform in-office procedures such as simple skin surgery, fracture care, joint injections, maternity care, IUDs, etc.
• Participate in quality improvement and population health initiatives.
• Educate and empower patients to take ownership of their health.

Required Qualifications:

• Unrestricted medical licensure in the state where this job will be performed or ability to obtain prior to starting employment

Compensation for this specialty generally ranges from $109,500.00 - $164,000.00. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
