Qualifications

• Current professional licensure as a Physical Therapist.
• Current CPR Certification.

Responsibilities

• Provide evaluation and establish a treatment plan with functional/work goals.
• Progress treatment and goals.
• Communicate with physicians in writing in a timely manner.
• Discharge planning and follow-up with patients.
• Supervise rehabilitation staff and PTAs when applicable.
• Document treatment and communications in patient records.
• Develop relationships with referral sources.
