Job summary

AdaptHealth is seeking an Advanced Care Specialist to guide patients through home medical equipment with a patient-centered approach and clear communication. You respond to patient needs to deliver the right products and services at the right time while aligning with organizational goals.

In this role you will send letters when contact cannot be made, answer emails promptly, verify delivery details, troubleshoot equipment over the phone, and coordinate with care teams to improve health outcomes.

Qualifications

  • Experience in health care administrative, financial, or insurance customer services, claims, billing, call center or management.
  • Senior level requires two years of work-related experience and one year of exact job experience.
  • Exact job experience in Medicare certified HME, Diabetic, Pharmacy, or home medical supplies environment that routinely bills insurance.

Responsibilities

  • Send letters to patients when contact cannot be made.
  • Consistently answer emails concerning patients’ needs in a timely manner.
  • Follow through on emails/phone calls until patient is taken care of.
  • Communicate appropriately and professionally with internal and external customers.
  • Make recommendations for equipment that will improve quality of care as appropriate.
  • Troubleshoot concentrator and other equipment problems over the phone.
  • Verify or obtain alternate contact information.
  • Verify delivery address, delivery instructions and telephone number for all orders.
  • Review documentation to ensure it is valid prior to processing an order.
  • Understand and utilize cost-effective delivery methods for items ordered.
  • Document accounts with any delivery expectations and requests.
  • Use standard note formats and notate contact with patient or family.
  • Choose the appropriate location for order routing.
  • Route non-care team orders to appropriate department or regional branch to fulfill.
  • Complete all orders received via CMB, email, fax, or phone in a timely manner.
  • Identify trends to improve efficiency and cost savings.
  • Assist in achieving departmental and organizational goals by helping with schedules and tasks.
  • Participate in team activities, mentoring, and training with coworkers.
  • Develop and maintain reference materials for staff and facilitate information sharing.
  • Escalate patient concerns to supervisor.

Advanced Care Specialist

The primary focus of the Advanced Care Specialist is to successfully guide patients through AdaptHealth using a patient centered approach and effective communication. She/he responds to patients needs in a way that are helpful and beyond expectation to provide an exceptional customer or patient experience at each interaction. The primary goal is to collaboratively work with patients to positively affect their health outcomes providing the right products and services at the right time while maintaining alignment with organizational goals and objectives.

Job Duties
  • Responsible for sending letters to patients when contact cannot be made.

  • Supports the achievement of departmental and organizational goals by assisting co-workers in completion of call schedules and other assigned tasks.
  • Is actively involved in team activities, evidenced by participation, mentoring, and training with co-workers.
  • Assists in the development and maintenance of reference materials for use by staff and facilitates the sharing of information.
  • Escalates patient concerns to supervisor.

Minimum Job Qualifications
