AgeWell is seeking a Nurse Practitioner to join our growing practice in Jacksonville, FL. The selected Advanced Practitioner Provider will serve as a link between inpatient and outpatient care as a member of the Baptist Health team.

Job Responsibilities

• Consults with patients about their current conditions, orders diagnostics, analyzes results, diagnoses conditions, and prescribes treatment strategies, including prescription medications.
• May also actively participate in research in the field and advocate for policies that support the needs of elderly patients.
