Grace at Home is seeking an APP Lead to train and manage Nurse Practitioners, Physician Assistants, and RNs delivering in-home and telehealth care. Lead a multidisciplinary team, ensure high-quality patient care, document visits, and optimize care navigation for members and families.
The role requires a Master’s in nursing, an active NP license, and 2+ years of clinical leadership with Medicare/Medicaid experience.

Reporting to the Regional Chief Medical Officer, the APP Lead is responsible for training and day to day management of Nurse Practitioners (NPs), Physician Assistants (PAs), and RN’s, ensuring effective collaboration, proper clinical management, documentation of visits, and high-quality patient care delivery to Grace at Home family members. This role involves team leadership, performance monitoring, overseeing an interdisciplinary team to meet organizational goals, improve member outcomes, and provide exceptional patient care. The assistant manager will also manage processes to ensure members, and their families, receive timely and effective navigation support, both telephonically and in the field. This position is accountable for ensuring Grace at Home is positioned to innovatively deliver on its promise. The APP Leads will have a strong knowledge base in providing care in the home, Value Based Care, HEDIS and Stars, documentation and coding, and medical practice principles. They will stay informed about advancements in healthcare practices and disseminate relevant knowledge.
In-office work is performed indoors in a traditional office setting with air conditioning, artificial light, and an open workspace.
In this position you will need to communicate with customers, vendors, management, and other co-workers in person and/or via phone or telehealth. Regular use of the telephone and e-email for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 10 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
