Job summary

Riveon is seeking an Outpatient Psychiatric Nurse Practitioner to perform comprehensive psychiatric evaluations, diagnose, treat, and manage medications for patients in our outpatient behavioral health program.

You will collaborate with a multidisciplinary team, document in the EHR, and develop personalized treatment plans while ensuring patient safety and quality care. Ohio licensure and APN prescriptive authority are required.

Qualifications

  • A Master’s degree in Nursing with NP prescriptive authority is required.
  • Certification as a Psychiatric Nurse Practitioner is required.
  • Current CPR certification and professional licensure in Ohio.
  • Bilingual abilities are an advantage but not required.

Responsibilities

  • Perform comprehensive psychiatric evaluations for new patients.
  • Establish or confirm psychiatric diagnoses and develop treatment plans.
  • Prescribe and monitor psychiatric medications and monitor safety.
  • Maintain accurate clinical documentation in the EHR.
  • Collaborate with interdisciplinary team and participate in quality initiatives.
  • Provide patient education and promote engagement in care.
  • Maintain licensure and adhere to policies.

POSITION PURPOSE AND OBJECTIVES

The Outpatient Psychiatric Nurse Practitioner provides comprehensive, patient-centered psychiatric evaluation, diagnosis, treatment, and medication management to individuals receiving outpatient behavioral health services. The Psychiatric Nurse Practitioner is responsible for assessing mental health needs, developing individualized treatment plans, prescribing and managing psychiatric medications within the scope of practice, and monitoring treatment response and patient safety.

ESSENTIAL JOB FUNCTIONS
  • Performs comprehensive psychiatric evaluations on new patients, including psychiatric history, mental status examination, medical history, medication history, substance use history and relevant psychosocial factors.
  • Establish or confirm psychiatric diagnoses
  • Incorporates the individualized new or revised medical treatment plan into the existing Electronic Health Record (EHR).
  • Provided psychiatric medication management and monitoring effectiveness, adherence, side effects, contraindications, drug interactions, and potential safety concerns.
  • Monitor for psychiatric stability and client safety and developing safety plans
  • Provide client education and promote engagement
  • Maintains accurate clinical documentation
  • Promote client safety and quality by participating in quality improvement activities and utilize evidence-based practices.
  • Collaborate as interdisciplinary team member
  • Demonstrates professional accountability by maintaining required licensure and adhere to organizational polices.
  • Conducts Peer Review activities as required.
  • Attends Psychiatric, Clinical and other Agency meetings as required.
  • Coordinate continuity of care by working collaboratively with staff to coordinate care to patients
  • Meets regularly with collaborating Psychiatrist to effectively communicate about cases
  • Achieves required productivity expectation.
  • Reacts to change and stress productively.
  • Maintains regular and predictable attendance and punctuality.
  • Gets along with others and works as a team.
  • Exhibits the knowledge, skills, abilities, and minimum requirements listed in this Position Description.
  • Other duties as assigned.

Hours Available: Full time, days, Monday-Friday

Requirements
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
  • In order to perform the essential functions of this job, after an orientation period, the employee must possess the following: a working knowledge of Riveon Policy and Procedures, including sensitivity and adherence to clients’ rights, confidentiality, and health and safety issues.
  • The employee must exhibit sensitivity to different cultures.
  • This employee must be up to date on all certifications, continuing education, and registration with the Ohio Board of Nursing which allows prescription authority under supervision in the State of Ohio.
WORKING CONDITIONS
  • The work is mainly provided in an office setting, although travel may be required dependent upon services contracted through other agencies such as medical, rehabilitation, or correctional settings. Because of the variety of environments and physical conditions that will be encountered, this individual must be able to successfully interact and navigate in all types of facilities and weather conditions.
  • This individual may periodically be required to lift and/or carry up to 20 pounds.
  • Must be able to physically interact with client and others in emergency situations to prevent harm to client and others.
  • The individual in this position must be able to communicate clearly both verbally and in writing.
  • Due to the extensive computer work this individual must be able to sit for extended periods of time and operate required equipment.
REQUIREMENTS/QUALIFICATIONS
  • A Master’s degree in Nursing and certification as an Advanced Practice Nurse with prescriptive authority.
  • Certification as a Psychiatric Nurse Practitioner is required.
  • CPR certified
  • The employee must possess exceptional oral and written communications skills.
  • Basic computer skills required, including familiarity with Windows and Microsoft Office.
  • The employee must be able to conduct business in a professional manner.
  • Must exhibit cultural sensitivity.
  • Bilingual abilities (especially, Spanish and English) are not required, but a great advantage for this position.
  • Favorable references and/or evaluations.
