Job summary

Mesa County Department of Human Services in Grand Junction, CO is seeking a Social Case Worker to provide assessment and case management services to elderly and disabled adults at risk of neglect, abuse, or exploitation.

Qualifications

  • Bachelor’s degree in a human behavior science field, OR 30 semester hours/45 quarter hours of qualified coursework in human behavioral sciences or health care related fields, such as social work, sociology, psychology, geriatrics, nursing, counseling, or related areas.
  • One year of professional casework experience after degree.
  • Experience working with adults is preferred.
  • Bilingual English/Spanish is desired.

Responsibilities

  • Provides one-on-one assessments of at-risk adults who are unable to protect their own interests.
  • Seeks court action to obtain protective services or guardianships in emergencies.
  • Collaborates with law enforcement and district attorney’s office in investigations of abuse, neglect, and exploitation.
  • Performs case planning for individuals requiring ongoing protective services.
  • Coordinates assessments and ongoing services with collateral agencies.
  • Attends multidisciplinary meetings and mandatory continuing education hours.
  • Provides outreach and education about adult protection services.
  • Documents assessments and actions throughout all phases of the case.
  • Maintains clear oral and written communication.
  • Performs other related duties as assigned.

JOB SUMMARY:
Provides assessment and case management services to elderly and disabled adults who are at risk of neglect, abuse, or exploitation.

ESSENTIAL DUTIES:

  • Provides one-on-one assessments of at-risk adults who are unable to protect their own interests based upon the following: a) physical appearance and hygiene; b) medical, nutritional, and environmental needs; c) competency and ability to understand self-determination and the result of the situation that is placing him/her at risk; d) activities of daily living; 3) support systems; f) ability to manage financial affairs.

  • Seeks court action to obtain protective services and/or potential guardianships in emergency situations.

  • Works closely with law enforcement agencies and district attorney’s office in investigations of abuse, neglect, and exploitation of at-risk elders. Works collaboratively with multiple community agencies to assess, investigate, and implement services for at-risk adults.

  • Performs case planning for individuals who require ongoing protective services which may include referral for services to reduce harm and risk.

  • Provides coordination of assessments and/or ongoing service delivery with appropriate collateral agencies.

  • May attend inter-disciplinary team meetings regarding client care, advocacy, and service coordination; attends conferences and meetings of various types to accrue mandatory continuing education hours.

  • Provides outreach, information and education to the community and organizations regarding adult protection services.

  • Documents assessments and actions taken throughout all phases of the case.

  • Oral and written communication must be clear.

  • Performs other related duties as assigned.

Minimum Requirements:

MINIMUM QUALIFICATIONS REQUIRED:

  • A bachelor’s degree from an accredited institution with a major in a human behavior science field, OR a degree with 30 semester hours, or 45 quarter hours, of qualified work in development in human behavioral sciences or health care related courses, such as, social work, sociology, psychology, psychiatry, gerontology, nursing special education, family intervention techniques, diagnostic measures, therapeutic techniques, guidance and counseling, criminal justice, or other human behavioral sciences, or a medical field relevant to the APS program and/or at-risk adults.

AND One year of professional casework in a public or private social services agency completed after the degree is obtained.

  • Experience working with adults is preferred.

Additional Information:

  • Offer of employment contingent on criminal history, MVR check and education verification. Must have valid driver’s license, if you are from out of state, you must obtain a valid Colorado driver’s license within 30 days of hire date, and you must not have any major violations, including DUI, DWI, and/or DWAI conviction(s) in the past three years.

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:

This position requires standing, stooping, sitting, bending, twisting, and lifting up to 10 pounds.

Work is generally confined to a standard office environment.

Employee may exposed to disease of known or unknown origin, blood borne pathogens, body fluids and tissue, the threat of indirect violence/conflict by other individual, and other unanticipated circumstances associated with the position.

Employee may be exposed to fumes, noxious odors, dusts, mists, gases, and poor ventilation that affect the respiratory system, eyes, or the skin.
