Carease Health – PA is seeking an experienced Adult Nurse Practitioner or Physician Assistant to deliver preventative care to residents in skilled nursing facilities. The role combines on-site visits with remote monitoring, supported by a dedicated scribe team to streamline documentation.

As an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.
