Job summary

Northwestern Medicine is seeking a Case Therapist I in Illinois to deliver clinical services including group, individual, and family counseling under supervision. The ideal candidate holds a Master's in social work, psychology, or counseling and licensure within 12 months (LPC/LSW or equivalent).

Qualifications

• Master's degree in social work, psychology, counselling, or related field.
• Licensed Professional Counsellor (LPC) or Licensed Social Worker (LSW) within twelve months of hire.
• Two years post graduate experience; experience with substance abuse and mental health populations.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Provide direct clinical services, primarily group therapy, along with individual and family counseling.
• Coordinate care by communicating with the treatment team, patients, and families.
• Maintain accurate and timely documentation in the electronic medical record.
• Support discharge planning and ensure continuity of care across the treatment spectrum.

Qualifications

Education:

Master's degree in social work, Psychology, Counselling, or related field

Licensure:

Licensed Professional Counsellor (LPC), Licensed Social Worker (LSW) within twelve months of hire

Experience:

Two years post graduate experience

Experience working with substance abuse and mental health populations

Preferred:

• Two to five years’ experience and training in assessment, individual, family, and group counselling
• Three to five years in intensive treatment setting and /or mental health setting
• Certified Alcohol and Drug Counsellor (CADC)
• Licensed Social Worker (LSW), or Licensed Professional Counsellor (LPC), or Licensed Clinical Professional Counsellor (LCPC), or Licensed Clinical Social Worker (LCSW), or Certified Alcohol and Drug Counsellor
