Active Generations is seeking an ADS Program Coordinator in Sioux Falls to support person‑centered experiences and engaging daily activities for participants. You will coordinate monthly calendars, lead activities, and assist ADS Specialists in delivering high‑quality care.

You will collaborate with ADS leadership and families to inform care planning, document progress, and ensure smooth operations across shifts.

Qualifications

  • Two years of experience in activity planning, recreational programming, or program coordination.
  • Experience working with older adults, including those living with Alzheimer’s disease or other forms of dementia, is preferred.
  • Strong organizational and relationship‑building skills with the ability to plan and facilitate engaging activities.
  • Clear verbal and written communication, including accurate participant documentation.

Responsibilities

  • Support daily program operations by helping ensure participant and program needs are met during scheduled shifts.
  • Serve as a point of contact for staff on shift to assist with questions, participant needs, and activity coordination.
  • Communicate participant updates or concerns to appropriate team members, including ADS leadership, families, and caregivers as directed.
  • Develop and maintain monthly activity calendars aligned with participant interests, needs, and abilities.
  • Lead group and individual activities and support ADS Specialists in facilitating engaging programming.
  • Provide coaching and guidance to staff during activities to help encourage participant engagement and adapt programming when needed.
  • Assist participants with Activities of Daily Living (ADLs) and other personal care needs.
  • Observe and document participant preferences, behaviors, and changes to inform care planning and program adjustments.
  • Complete required documentation, including daily notes, monthly charting, and incident reports.
  • Assist ADS leadership with onboarding and training support for new ADS Specialists.

SUMMARY:

As the ADS Program Coordinator, you will play a hands‑on role in supporting an engaging, person‑centered experience for participants. This position coordinates monthly activity programming, leads and supports daily activities, assists ADS Specialists in delivering high‑quality care, and serves as a point of contact for families and caregivers. The ADS Program Coordinator works closely with ADS leadership to help ensure smooth daily operations and meaningful participant engagement.

ESSENTIAL FUNCTIONS:
  • Support daily program operations by helping ensure participant and program needs are met during scheduled shifts.
  • Serve as a point of contact for staff on shift to assist with questions, participant needs, and activity coordination.
  • Communicate participant updates or concerns to appropriate team members, including ADS leadership, families, and caregivers as directed.
  • Develop and maintain monthly activity calendars aligned with participant interests, needs, and abilities.
  • Lead group and individual activities and support ADS Specialists in facilitating engaging programming.
  • Provide coaching and guidance to staff during activities to help encourage participant engagement and adapt programming when needed.
  • Assist participants with Activities of Daily Living (ADLs), including but not limited to toileting, showers, grooming, mobility, and other personal care needs.
  • Observe and document participant preferences, behaviors, and changes to help inform care planning and program adjustments.
  • Complete required documentation, including daily notes, monthly charting, and incident reports, in a timely and accurate manner.
  • Assist ADS leadership with onboarding and training support for new ADS Specialists.
  • Promote a positive, respectful, and team‑oriented environment that reflects Active Generations’ values and commitment to service excellence.
  • Collaborate with ADS leadership and other departments to support participant well‑being and program success.
  • Assume responsibility for program flow during assigned shifts in the absence of ADS leadership.
EDUCATION & EXPERIENCE:
  • High School Diploma or GED
  • Additional education in health, human services or related field is preferred
  • CPR/AED and First Aid Certification
KNOWLEDGE, SKILLS & ABILITIES:

A minimum of two (2) years of experience in activity planning, recreational programming, or program coordination. Experience working with older adults, including individuals living with Alzheimer’s disease or other forms of dementia, is preferred. Strong organizational and relationship‑building skills with the ability to plan and facilitate engaging group and individual activities. Demonstrated ability to communicate clearly both verbally and in writing, including completing accurate participant documentation. Ability to observe participant behaviors and respond with patience, empathy, and sound judgment. Basic understanding of aging, Alzheimer’s disease, and dementia‑related conditions, including the ability to use person‑centered engagement techniques and adapt activities to meet varying physical and cognitive abilities. Shows initiative, adaptability, and flexibility in a dynamic, team‑based care environment.

PHYSICAL REQUIREMENTS & WORKING CONDITIONS:
  • Ability to stand and walk for extended periods of time throughout the shift.
  • Ability to assist participants with mobility, including transfers, positioning, and ambulation support.
  • Ability to occasionally lift or assist in lifting up to 50 pounds with appropriate support or equipment.
  • Ability to bend, reach, kneel, crouch, and perform physical tasks necessary to assist participants with activities of daily living (ADLs).
  • Ability to safely assist participants using mobility devices such as walkers, wheelchairs, or gait belts.
  • Ability to facilitate activities that may involve movement, set‑up of equipment, and repositioning furniture or supplies.
  • Ability to respond quickly to participant needs or emergency situations.

Work is performed primarily in an Adult Day Services program environment that supports older adults with varying physical abilities, health conditions, and cognitive needs, including individuals living with Alzheimer’s disease or other forms of dementia. The role involves frequent interaction with participants, families, caregivers, volunteers, and staff in a dynamic and service‑oriented setting. The environment may include moderate noise levels from group activities and daily programming. Employees may be exposed to bodily fluids when assisting with personal care and must follow established safety and infection control procedures. The position requires active movement throughout the day, attention to participant safety, and the ability to adapt to changing participant needs. Work schedules may include daytime hours, occasional special events, and participation in staff meetings or trainings as needed.
