Job summary

Job description

Summary Statement

This position is located within the Child and Family Care Coordination (CFCC) unit and works with youth with more intensive behavioral health treatment needs and their families. The role focuses on engaging families by assisting them in developing a plan of care for their children, identifying, referring, and linking to appropriate behavioral health treatment, leveraging natural and community supports, coordinating services, monitoring progress, and providing updates to other systems including the court when applicable. Work hours may occasionally extend beyond the traditional 8:00 am – 4:30 pm workday.

Essential Functions
  • Participates in treatment planning and discharge planning with facility personnel and other agencies to ensure expeditious release of the adolescent patient.
  • Designs aftercare treatment plans and refers clients to appropriate outpatient, residential, and community support facilities, ensuring discharge recommendations and aftercare plans are implemented.
  • Interacts with clients’ families, interviews members, observes family dynamics, assesses strengths or dysfunctions, and adapts aftercare treatment plans to reflect their needs.
  • Provides direct supportive services including home visits, individual and family counseling, and family crisis intervention.
  • Monitors outpatient treatment components to ensure progress and resolves any arising problems.
  • Develops and maintains a community resources “bank” to provide necessary services for adolescents and families.
  • Prepares monthly statistical, narrative, and court reports detailing caseload activities.
  • Prepares progress notes on each client.
  • Participates in staff meetings.
  • Makes formal presentations before community groups, schools, and/or professional organizations.
Job Requirements
  • Possession of a Bachelor’s degree or higher in Behavioral or Social Science or related field.
  • Minimum six months of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet individuals’ human service needs.
  • Minimum six months of experience recommending clinical treatment, counseling, or determining eligibility for health or human services/benefits as part of a client’s service plan.
  • Minimum six months of experience interpreting laws, rules, regulations, standards, policies, and procedures.
  • Minimum six months of experience in narrative report writing.
  • Possession of a valid Driver’s License (not suspended, revoked, or cancelled).
