Job summary

Universal Hospital Services Inc. is seeking a Social Worker to support patients' psychosocial needs and emotional health. You will facilitate treatment planning, conduct assessments, provide psycho-education, offer short-term therapy, lead group sessions, and coordinate discharge planning with community resources.

Eligible candidates include AAC or LSWAIC credentials. BLS, First Aid, and Handle with Care are required; strong de-escalation skills are essential for effective patient care.

Qualifications

• Basic Life Support (BLS) and First Aid certifications required

• Verbal de-escalation skills required

Responsibilities

• Conduct psychosocial assessments to evaluate patients' emotional, behavioral, developmental, social, and support needs.
• Provide individual, family, and group interventions to support treatment goals and patient progress.
• Develop, implement, and update individualized treatment plans in collaboration with patients, families, and the interdisciplinary treatment team.
• Facilitate treatment planning meetings and coordinate care with physicians, nursing staff, and other clinical disciplines.
• Assist patients and families with identifying resources and addressing psychosocial, educational, financial, and discharge planning needs.
• Monitor patient progress, evaluate treatment effectiveness, and adjust interventions as appropriate.
• Facilitate family meetings and support ongoing communication between patients, families, and the treatment team.
• Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, and discharge planning activities.
• Ensure confidentiality and compliance with all applicable regulatory, accreditation, and organizational standards.
