Job summary

VITAS Healthcare is seeking an Admissions Nurse to bridge hospital or community care with hospice, guiding patients and families through a pivotal transition with empathy and clinical confidence.

As a dedicated RN, you will assess eligibility, educate on Medicare benefits, and coordinate admissions while collaborating with a multidisciplinary team to ensure dignified, patient-centered care.

Qualifications

• Registered Nurse with a valid state license.
• Minimum 2 years nursing experience; hospice, palliative, ER, ICU, home health or case management preferred.
• Strong clinical assessment and critical thinking skills.
• Compassionate communicator with excellent interpersonal skills.
• Active listener who builds trust and rapport.
