This role emphasizes seamless transitions, thorough assessments, thorough documentation, and collaboration with physicians and the care team to deliver resident-centered care and a meaningful difference each day.

Qualifications

• Current RN license in New York State.
• Strong clinical assessment and communication skills.
• Excellent organizational and documentation skills.
• Compassionate and resident-focused approach.
• Ability to work effectively as part of a team.
• Long-term care or admissions experience preferred.

Responsibilities

• Complete comprehensive nursing assessments for new admissions
• Coordinate and facilitate the admission process, ensuring a smooth transition for residents and their families
• Review medical records and clinical information to develop individualized plans of care
• Collaborate with physicians, residents, families, and the interdisciplinary team to deliver compassionate, resident-centered care
• Ensure accurate and timely documentation of all patient information
• Provide exceptional care and support to residents, prioritizing their physical, emotional, and social well-being
• Other duties as assigned by the DON or designee
