BAYADA Home Health Care in Burlington County, NJ is seeking a Full-Time Admissions Physical Therapist (PT) to perform OASIS SOC visits for adult and geriatric home health patients. You will evaluate mobility and function, develop individualized care plans, and collaborate with the clinical team to ensure safe, effective home-based care.

The role emphasizes flexible scheduling, independent work, and ongoing professional development within a supportive clinical team.

Qualifications

• Must hold a New Jersey PT license.
• Minimum of one year of recent experience as a Physical Therapist.
• Prior home health experience required.
• OASIS experience required.
• HCHB experience strongly preferred.
• Strong clinical assessment, communication, organizational, and time-management skills.
• Ability to work independently and effectively manage a flexible schedule.
• Reliable transportation and ability to travel throughout Burlington County.

Responsibilities

• Complete OASIS Start of Care (SOC) admissions visits for home health patients throughout Burlington County.
• Perform comprehensive physical therapy evaluations, including mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Develop individualized treatment plans based on patient needs and physician orders.
• Collaborate with nursing and interdisciplinary team members to coordinate comprehensive patient care.
• Educate patients, families, and caregivers to promote independence, safety, and optimal outcomes.
• Accurately document clinical findings, assessments, and services using electronic medical records (EMR) and tablet technology.
• Ensure timely and accurate completion of OASIS documentation and admission requirements.
• Continuously evaluate patient needs and make appropriate recommendations for the ongoing plan of care.

Additional Information:

Base Pay, depending upon qualifications, status, and subject to negotiation:

$63.00-$73.00 per point
