Job summary

The Lutheran Home of Southbury in Southbury, CT is seeking an Admissions Nurse (RN) to conduct pre-admission and admission assessments, review records for appropriateness, and coordinate admissions with hospitals, physicians, and families. You will ensure a smooth transition for new residents and maintain compliant clinical documentation.

Ideal candidates have CT RN licensure, experience in skilled nursing or geriatric care, strong assessment and communication skills, and comfort with EMR

Qualifications

• Current Connecticut RN License in good standing.
• Previous experience in skilled nursing, long-term care, rehabilitation, case management, admissions, or geriatric nursing preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, customer service, and relationship-building abilities.
• Experience with electronic medical record systems.
• Ability to prioritize and manage multiple responsibilities in a fast-paced healthcare environment.
• Passion for providing exceptional service to residents and families.

Responsibilities

• Conduct pre-admission and admission nursing assessments.

• Facilitate a smooth transition for new residents entering our community.
• Complete and maintain accurate clinical documentation in compliance with regulatory standards.
• Communicate resident care needs to nursing and interdisciplinary teams.
• Provide education, support, and guidance to residents and families throughout the admission process.
• Collaborate with referral sources and healthcare partners to support census growth and resident-centered care.

Schedule

Monday-Friday Evening Shift

• 3:00 PM - 11:00 PM or
• 4:00 PM - 10:00 PM
