Job summary

American Senior Communities is seeking an Admission Nurse - RN for a management-oriented role with a 10am-6pm schedule and on-call rotation. You will oversee resident care transitions, monitoring admissions and coordinating care plans across shifts in a geriatric long-term care setting.

The role emphasizes leadership, staff education, and ensuring quality care in alignment with CARE values. Indiana RN licensure and LTAC management experience are required.

Qualifications

• Graduated from an accredited school of nursing and hold a current Indiana Registered Nurse license.
• Minimum of three years’ clinical experience in Geriatrics/Gerontology with at least one year as a Long-Term Care Departmental Manager.
• Prior consulting experience preferred.

Responsibilities

• Coordinate resident care between shifts to ensure continuity of care.
• Monitor admissions to ensure care plan interventions are in place across shifts.
• Coordinate and manage evening/weekend discharges.

Resident Care Coordinator – Registered Nurse (RN)

Essential Position Functions

• Coordinates resident care between shifts to ensure continuity of care, and exercises discretion when change of
  care is needed and communicates accordingly to those involved in the care of the resident(s).
• Monitors new admissions to ensure appropriate care plan interventions are in place and consistent from shift to
  shift.
• Ensure evening admissions are effectively transitioned into health care community and that all resident needs
  are met during first few hours or care.
• Coordinates and manages evening and/or weekend discharges to ensure that appropriate interventions have been
  implemented and that if a discharge must take place that it happens in a manner that is conducive to the health
  and well being of the resident.
• Provides education and training to direct care givers of residents experiencing changes in condition to ensure
  continuity of care from shift to shift.
• Monitors resident experiencing changes in condition; residents that have experienced a recent fall; residents
  with new or worsening behaviors; residents that require increased nursing time related to medications/treatment
  changes to ensure continuity of care from shift to shift.
• Assists the Director of Nursing Services in the daily functions of clinical services as directed by the DNS. This
  includes, but is not limited to, rotating weekend call, assisting with daily follow-through of resident change of
  condition reports, supervision of resident dining service, assisting with resident complaint investigation and
  resolution, investigation of abuse allegations, etc.
• Demonstrates C.A.R.E. values to our residents, family members, customers and staff.
• Complies with the facilities privacy practices/procedures and all state/federal privacy laws as outlined by
  HIPAA related to resident and employee records, to include but not limited to information accessed through any
  company web-based system and/or electronic medical records.
• Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are
  provided to the residents we serve.
• Complies and adheres to the appropriate use of Personal Protective Equipment (PPE) required by the
  Bloodborne Pathogens Standards. Protective Personal Equipment (PPE), including personal protective
  equipment for eyes, face, extremities, protective clothing, and protective shield and barriers, will be provided,
  used, and maintained.
