Life Care Centers of America is seeking an Admissions Nurse (Licensed Practical Nurse) to provide compassionate, high-quality care during patient admissions and readmissions in our long-term care facilities in Hawaii.

The role requires an active LPN/LVN license, completion of an accredited LPN program, CPR certification, and a focus on accurate documentation, timely communication with physicians, and maintaining patient safety and dignity in a fast-paced, team-driven environment.

Qualifications

• Attended an accredited LPN program.
• Active LPN/LVN license in good standing.
• CPR certification required on hire or during orientation.
• Geriatric nursing experience preferred (1 year).

Responsibilities

• Provide hands on admissions assessments and care.
• Communicate assessment findings with the physician.
• Deliver quality nursing care during admission and readmission.
• Evaluate needs and initiate care plans at admission/readmission.
• Receive, document, and transcribe orders accurately.
• Relate orders to care team timely for medications and implementation.
• Chart timely and accurately.
• Exhibit excellent customer service and maintain professionalism.
• Assist in patient evacuation as needed.
• Communicate effectively within an interdisciplinary team.
• Read, write, speak, and understand English.
