Job summary

Hospice Savannah is seeking a Clinical Intake Representative to manage referrals for admission and coordinate intake processes. You will review clinical records for eligibility, verify payer sources, obtain pre-certifications, and handle incoming calls while scheduling visits and educating patients and families on services.

Qualifications

  • Graduate of an accredited school of Nursing.
  • Licensed Practical Nurse (LPN) license required.
  • Experience in hospice, oncology, or home health preferred.
  • Knowledge of Joint Commission standards and state and federal regulations regarding care and reimbursement required (hospice-specific knowledge preferred).
  • CPR certification required (Certified Instructor preferred).
  • Previous experience in clinical education and planning/preparing educational in-services preferred.

Job Summary

The Clinical Intake Representative is responsible for managing all intake processes required to efficiently process referrals for admission to Hospice Savannah, Inc. services. This includes reviewing all clinical records for eligibility, reviewing payer sources, obtaining all pre-certifications, managing incoming calls, scheduling visits and education patients and families on services within Hospice Savannah, INC When appropriate. The Clinical Intake Representative is committed to the philosophy, goals and objectives of the Hospice Savannah concept while consistently demonstrating the Hospice Savannah values and compliance with organization’s services excellence standards.

Qualifications
  • Graduate of an accredited school of Nursing.
  • Licensed Practical Nurse (LPN) license required.
  • Experience in hospice, oncology, or home health preferred.
  • Knowledge of Joint Commission standards and state and federal regulations regarding care and reimbursement required (hospice-specific knowledge preferred).
  • CPR certification required (Certified Instructor preferred).
  • Previous experience in clinical education and planning/preparing educational in-services preferred.
  • Valid driver’s license, automobile insurance, and reliable transportation required.

  • Demonstrated knowledge of clinical interventions, treatments, pain management, and symptom management; assist the RN in maintaining accurate medication administration records.
Physical Demands and Work Environment
  • Requires moderate to intense visual concentration, reading, computer use, and review of patient charts.
  • Daily contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers, and Hospice Savannah staff; requires professional demeanor, excellent communication, and problem-solving skills.
  • Risk of exposure to infectious diseases; requires skill in standard safety precautions.
  • Requires mobility to ambulate or lift a patient (with or without assistance devices) in excess of 75 pounds using proper body mechanics.
  • Encounters stressful situations with patients and families regarding end-of-life care; may be required to provide care in high-risk crime areas and potentially unsafe situations.
Position Type and Expected Hours of Work

This position requires moderate to intense visual concentration, including frequent reading, computer use, and detailed review of patient charts. The role involves daily interaction with patients, families, medical facilities, physicians, physician office staff, vendors and suppliers, and Hospice Savannah staff, and therefore demands a consistently professional demeanor, strong communication skills, and effective problem-solving abilities. The employee may be exposed to infectious diseases and must demonstrate competence in the use of standard safety precautions. The position also requires sufficient mobility to ambulate or lift patients, with or without assistive devices, in excess of 75 pounds while using proper body mechanics. In addition, the employee regularly encounters emotionally stressful situations related to end-of-life care and may be required to provide services in high-crime or potentially unsafe environments.

Travel

Must be able to travel as required, including occasional commuting between Hospice Savannah facilities utilizing a personal vehicle.
