Job summary

Castor Home Nursing Inc. in Sterling, IL seeks an experienced Home Health Admission RN to initiate Start of Care admissions and perform skilled nursing visits.

You will supervise LPNs in your region on 30-day cycles and complete recertifications after 30 days of care or post-hospitalization. The ideal candidate has an active Illinois RN license, Oasis experience, strong clinical judgment, and the ability to manage a high-volume documentation workflow with accuracy and efficiency.

Job Summary:
The Home Health Admission RN is responsible for initiating patient care through Start of Care (SOC) admissions, conducting supervisory visits for LPNs in their assigned region every 30 days, and completing recertifications following 30 days of care or post-hospitalization. This role also includes providing skilled nursing visits. The ideal candidate demonstrates strong clinical judgment, organizational skills, and the ability to manage a high volume of documentation with precision and efficiency.
