Job summary

Elara Caring is seeking an Admission Nurse (RN) to be the initial clinical touchpoint for patients welcoming them into home health care. You will assess, plan, and coordinate care to set up successful home-based treatment in collaboration with physicians and the interdisciplinary team.

The role requires an active RN license in NH, a minimum of one year of nursing experience, and willingness to travel about 50% with reliable transportation.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted RN license in the state of employment.
• Minimum of one year of clinical nursing experience.
• Ability and willingness to travel throughout the assigned service area (approximately 50%).
• Reliable transportation, valid driver's license, and current auto insurance meeting state requirements.
• Ability to lift, push, pull, and support up to 50 pounds.

Responsibilities

• Complete comprehensive admission assessments to evaluate patients' needs.
• Develop and initiate individualized plans of care with physicians and the care team.
• Perform initial nursing evaluations and ongoing reassessments as needed.
• Accurately document patient assessments, findings, and care plans in the EMR.
• Communicate changes in patient condition promptly with physicians and the care team.
• Ensure compliance with HIPAA regulations and clinical standards.
• Identify safety concerns and report findings to leadership.
• Promote continuity of care and positive patient outcomes.
• Support branch operations with additional responsibilities as assigned.

As An Admission Nurse, You Will

• Complete comprehensive admission assessments to evaluate patients' physical, emotional, psychosocial, and environmental needs
• Develop and initiate individualized plans of care in collaboration with physicians and the interdisciplinary care team
• Perform initial nursing evaluations and ongoing reassessments as needed
• Accurately document patient assessments, clinical findings, and care plans in the electronic medical record
• Communicate changes in patient condition promptly with physicians, Clinical Team Managers, and other members of the care team
• Ensure compliance with HIPAA regulations, company policies, and clinical standards
• Identify safety concerns and report significant findings to appropriate leadership
• Promote continuity of care while supporting positive patient outcomes
• Perform additional responsibilities as assigned to support patient care and branch operations

Requirements

• Graduate of an accredited nursing program
• Current, unrestricted Registered Nurse (RN) license in the state of employment
• Minimum of one (1) year of clinical nursing experience
• Ability and willingness to travel throughout the assigned service area (approximately 50%)
• Reliable transportation, valid driver's license, and current auto insurance meeting state requirements
• Ability to lift, push, pull, and support up to 50 pounds, including assisting with patient transfers and moving equipment

Preferred Qualifications

• Associate or bachelor's degree in nursing (ADN or BSN)
• Previous Home Health or Hospice experience
• Experience using Homecare Homebase (HCHB)
