Job summary

MJHS seeks an Admission Nurse to initiate and resume high-quality home health services for patients referred to the agency in Queens, NY and occasionally Brooklyn.

You will create the initial, interdisciplinary plan of care in collaboration with physicians or approved practitioners, ensure regulatory compliance, and provide expert community health nursing care and teaching to patients in their homes.

Qualifications

• Diploma in Nursing required; NY RN license in good standing.
• Minimum one year of clinical experience in a Certified Home Health Agency.
• Knowledge of home health regulatory requirements and documentation practices.

Responsibilities

• Create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the plan in collaboration with physician or authorized practitioner (AP).
• Ensure agency compliance with regulatory and organizational requirements.
• Demonstrate expert knowledge unique to the practice of community health nursing.

• Knowledge of Microsoft Office and experience documenting in a clinical software system preferred.
• Driver’s License required in all boroughs except Manhattan.
• Strong assessment skills, clinical judgment, and critical thinking abilities.
• Ability to work independently in a community setting.
