Community Healthcare of Texas is seeking an Admission Nurse in Fort Worth to meet with patients and families, explain hospice services, and coordinate admission.

The role requires Texas RN, a valid driver’s license, and 1 year nursing experience; hospice case management is preferred. The position offers a $5,000 sign-on bonus along with competitive pay and comprehensive benefits.

Qualifications

• Strongly prefer 2+ years of experience in hospice case management
• Must have proficient computer skills and Microsoft Office suite experience

Responsibilities

• Meet with patients and families to explain the scope of hospice services and coordinate admission of patients.
• Conduct thorough admission assessments and establish initial nursing visit frequency, DME/RX needs, and bereavement/support services as needed.
• Attend Interdisciplinary Team meetings and report patient status.
• Complete paperwork and charting thoroughly and accurately according to policy.
• Possibly conduct nursing or 'tuck-in' visits and train admission staff on admission processes.
• May conduct supervisory visits on LVN or CNA staff.

POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services and coordinate admission of patients.

ESSENTIAL FUNCTIONS

• Conducts consultations for admission of patient services, including education of hospice services to patients and their families.
• Completes thorough assessment of patient at the time of admission; establishes/arranges initial frequency of nursing visits, DME/RX needs, and assesses the need for bereavement, social work, chaplain, and volunteer services.
• Attends and reports to the Interdisciplinary Team regarding the patient.
• Completes paperwork and charting thoroughly and accurately according to Policy.
• May conduct nursing or 'tuck-in' visits.
• May train other admission staff on admission processes.
• May conduct supervisory visits on LVN or CNA staff.
