Job summary

UMass Memorial Health in Webster, MA is seeking a Patient Navigator to connect patients with substance use disorder services within the Bridge Clinic and across the health system.

You will perform short-term case management, link patients to resources, coordinate referrals, transportation, and follow-ups to promote continuous care. Strong communication and organization are essential; a Bachelor’s degree is required, with CADC/LADC preferred.

Qualifications

• Experience with substance use disorders and mental health disorders preferred or willingness to learn.
• Two years working within the substance use disorders field.
• Bachelor’s Degree required.
• CADC or LADC certification preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Identify patient eligibility for grant criteria via access through the Emergency Department.
• Data collection and entry.
• Build community relationships with outside referral sources.
• Provide case management and referrals to meet patient needs.
• Connecting patients to the UMass Harrington Behavioral Health continuum of care.
• Expedite services and educate patients on clinic services or external referrals.
