Qualifications

• RN license or eligible to practice in Kansas
• Assoc. degree in nursing or RN diploma required

Responsibilities

• Coordinate rehabilitative care for Rehab patients.
• Educate patients and families about care plans and discharge criteria.
• Collaborate with Rehab Program Director and team to optimize patient outcomes.
• Ensure documentation and communication across care teams.
