Job summary

Qualifications

Responsibilities

• Evaluates assigned patients, gathering data from records, interviews, and team members.
• Develops and implements individualized PT treatment plans.
• Reviews plan of care with patients and communicates progress.
• Instructs patients and families in home programs or exercises.
• Provides direct therapy services across settings, coordinating with care plans.
• Maintains documentation such as evaluation results, plans, and progress notes.
• Communicates status and needs to patients, families, and the care team.
• Oversees PT assistants, techs, and students to ensure safe, effective care.
