Job summary

LiveWell Homecare Agency is seeking a licensed RN to provide direct patient care in a night-shift home health setting. You will assess, plan, implement, and evaluate care in cooperation with patients and families.

Responsibilities include developing individualized care plans, evaluating needs, communicating across care continuum, and maintaining safety and quality in a role with opportunities for career ladder advancement and pay progression as responsibilities increase.

Qualifications

• RN license required.

Role Overview: Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards. Promotes patient and family-centered care, educating on illness management, post-treatment care, and medication administration. Participates in activities that enhance patient safety, quality, and compliance, while engaging in ongoing professional development.
