Job summary

LiveWell Homecare Agency seeks a Registered Nurse to provide direct patient care in an acute care setting, including assessment, planning, implementation, and evaluation while adhering to nursing standards.

Role Overview: Provides direct patient care within an acute care setting, including assessment, diagnosis, planning, implementation, and evaluation, ensuring adherence to nursing standards. Promotes patient and family-centered care, educating on illness management, post-treatment care, and medication administration. Collaborates with an interprofessional team, developing and documenting individualized care plans, communicating effectively across the continuum of care, and evaluating physical, psychosocial, and emotional needs while respecting patient preferences and cultural values.

Responsibilities:

• Ensure patient safety, quality, and regulatory compliance through active participation and professional development.
• Assess patient needs, develop care goals, and coordinate with team members and families to implement plans.

Qualifications:
