Job summary

Qualifications

Responsibilities

• Perform patient assessment and reassessment with comprehensive change of shift reports.
• Assess patient learning needs and readiness for discharge, and ensure patient/family can manage after discharge.
• Complete documentation accurately and timely in paper and electronic formats.
• Provide direct patient care and assist physicians with procedures as needed.
• Report changes in patient condition to physician.
