Job summary

UAB Medical West Hospital is seeking a Social Worker to perform psychosocial assessments and participate in the treatment team. The role includes individual/family conferences, admission screenings for transition of care, and serving as a liaison to community resources including nursing homes, home health, hospice, and transportation.

Requirements include a Master's in Social Work, Alabama social work licensure, and BLS. LGSW is required; LCSW is a plus.

Qualifications

• Master's degree in social work from an accredited college or university.
• Alabama Social Workers license is required.
• LGSW required; LCSW is a plus.
• Basic Life Support certification by the American Heart Association (or within 30 days of hire).
• Minimum six months acute care experience preferred.

Responsibilities

• Perform psychosocial assessments and participate in treatment team meetings.
• Provide individual and/or family conferences and screen patients on admission for transition of care.
• Act as liaison with community resources/agencies and report abuse/neglect to DHR.
• Maintain current list of community agencies/resources and link patients to placements and services.
• Coordinate plans and organize comprehensive social work services for assigned patients.
