Qualificações

• Master's Degree in Social Work (MSW) from a CSWE-accredited university.
• Minimum 1 year hospital acute care or other related healthcare experience; or completion of a 1-year acute care MSW internship.

Responsabilidades

• Assess patients and support systems for discharge planning and psychosocial counseling needs.
• Establish plans for effective management of identified needs; perform assessment, planning, intervention and reassessment in a collaborative environment.
• Lead identification and management of patients with catastrophic needs and social determinants of health; collaborate with hospital and community service providers.
