Job summary

Actalent is seeking an RN Case Manager for a contract-based role in Loma Linda, CA. The ideal candidate coordinates patient care across acute and observational levels, develops discharge plans, and works with a multidisciplinary team to ensure appropriate utilization of resources.

Expect to collaborate with utilization review nurses and case management assistants to support post-acute referrals. The position requires an active California RN license, BSN, and at least 2 years of case management

Qualifications

• Active and unencumbered California RN license required.
• Current BLS certification.
• BSN degree required.
• Minimum 2 years case management in Level I trauma or inpatient setting preferred.
• 2+ years inpatient acute care experience in non-emergency department.
• 1 year hospital case management or utilization management experience.
• Experience with discharge planning in inpatient acute care.
• Strong knowledge of case management and utilization management practices.
• Familiarity with state/federal healthcare regulations in case management.

Responsibilities

• Perform comprehensive initial case management assessments for inpatients and observation patients.
• Develop safe, viable discharge plans with patients and families.
• Coordinate care across physicians, nurses, therapists, social workers, and other team members.
• Participate in multidisciplinary rounds to communicate needs and discharge plans.
• Support authorization of care with utilization review nurses from admission to discharge.
• Assist with post-acute referral processes and placement decisions.
• Apply nursing process in patient-centered case management.
• Ensure medical necessity criteria are met and documented.
