Proforma Health Inc. is seeking an Acute Hospital Physical Therapist for per diem coverage in a fast-paced acute care setting. You will evaluate medically complex and post-surgical patients and implement mobility-focused interventions to improve function and safety.

In this role, you will develop evidence-based treatment plans, document care in the EMR, and collaborate with physicians, nurses, and case managers to support smooth discharge planning. Strong communication and teamwork are essential.

Qualifications

• Licensed Physical Therapist.
• Strong clinical assessment and evidence-based treatment planning.
• Experience with complex post-surgical patients in acute care.
• Proficient in mobility training, gait, and transfers.
• Excellent communication, teamwork, and documentation skills.

Key Responsibilities
