Job summary

Yale New Haven Health seeks a hospital pharmacist to support patient care in an acute care setting. You will collaborate with physicians and nurses to ensure safe medication use, interpret clinical data, and provide drug information.

Require a B.S. or Pharm.D. from an accredited college, CT licensure (or eligibility), and about two years of hospital experience; residency is desirable but not required.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree.
• Residency in hospital pharmacy desirable but not required.
• Licensed to practice pharmacy in Connecticut (or eligible for reciprocation).
• Two years experience in an acute care hospital pharmacy is preferred for this position.
• Excellent communication skills and ability to work with others.

Responsibilities

• Resolves drug distribution problems assuring accurate and timely dose delivery to all patients.
• Interprets clinical and pharmacokinetic lab data to formulate medication monitoring plans.
• Problem identification and resolution, documenting findings and follow-up.
• Provides drug information to nurses, MDs, and other health care professionals
• Participates in educational forums within the department
• Documents medication use variances (MUVs) and adverse drug reactions (ADRs)
• Adheres to regulatory and safety quality standards as part of a High Reliability Organization.
