Job summary

Qualifications

• PharmD or BS in Pharmacy from an accredited school is required.
• Active and unrestricted Pharmacist license in state of practice.
• BLS certification required.
• Sterile Products Certification preferred.
• 1–2 years of clinical hospital pharmacy experience preferred.
• Experience in compounding sterile products and MTM preferred.

Responsibilities

• Review and verify medication orders for accuracy, dosage, and interactions.
• Prepare and dispense medications per protocols and physician orders.
• Compound sterile products, chemotherapy, and parenteral nutrition solutions aseptically.
• Provide drug information and consultation to healthcare professionals.
• Monitor patient medication profiles for contraindications and duplications.
• Educate patients on medication therapy and administration.
• Assist in inventory management and secure storage of controlled substances.
• Document clinical activities in the patient medical record.
• Support training of pharmacy technicians and students.
