Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to provide direct patient care in a 25-bed Acute Care Medicine unit. You will assess, plan, implement, and evaluate individualized care with a multidisciplinary team, promoting patient and family-centered care.

You will educate patients and families for post-discharge self-care, document care plans, and participate in ongoing professional development within a structured nurse career ladder that offers growth and compensation advancement.

Qualifications

• Associate's degree in nursing is required; Bachelor's degree preferred.
• Current RN licensure required.
• 2+ years of experience preferred (not required).
• Strong critical thinking, communication, and teamwork skills.
• Ability to work safely in a fast-paced environment.

Job description

This position involves providing direct patient care in a 25-bed Acute Care Medicine unit, managing patients with complex clinical conditions such as blood dyscrasias, endocrine disorders, heart and renal failure, and infectious diseases. The nurse will assess, diagnose, plan, implement, and evaluate individualized care plans in collaboration with a multidisciplinary team, including physicians, social workers, and therapists. Responsibilities include promoting patient and family-centered care, educating on illness management and post-discharge needs, ensuring safety and regulatory compliance, and communicating care plans effectively across the continuum.

Qualifications & Skills:

• Minimum of an Associate’s Degree in Nursing; Bachelor’s preferred.
• Current RN licensure required.
• No prior experience necessary, but 2+ years of experience is preferred.
• Strong critical thinking, communication, and teamwork skills.
• Ability to work safely and adapt to a fast-paced environment.
