Job summary

LiveWell Homecare Agency, LLC is seeking a registered nurse to provide direct patient care in an acute care setting, developing and documenting individualized care plans and educating patients and families. You will collaborate with an interprofessional team to ensure safe, high-quality care across transitions.

Ideal candidates hold RN licensure, with a nursing diploma/associate’s degree; a BSN is preferred and two years of experience with supervisory exposure is desirable.

Qualifications

• RN licensure is mandatory.

Responsibilities

• Assess and evaluate patient conditions, develop and implement care plans, and document progress.
• Educate patients and families on post-treatment care, medication, and disease management.
• Participate in safety, quality, and regulatory activities, and engage in ongoing professional development.
• Collaborate with multidisciplinary teams, including case managers, social workers, and physicians, to deliver comprehensive care.
