Job summary

TRINITY EMPLOYMENT SPECIALISTS seeks an experienced Acute Care NP/PA for Kansas City, MO. You will evaluate and treat acute illnesses, manage chronic conditions, and coordinate post-discharge care in a patient-centered outpatient setting that emphasizes quality and outcomes.

You will bring acute care or hospitalist experience, hold an active license, and maintain BLS/ACLS. Travel to nearby centers may be required; the role offers strong support and modern facilities with a comprehensive benefits

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Active, unrestricted state license and appropriate national certification.
• Current BLS and ACLS certifications.
• Acute care, hospitalist, emergency medicine, or complex internal medicine experience preferred.
• Experience managing medically complex patients with multiple acute and chronic conditions.
• Strong clinical assessment, diagnostic reasoning, and medical decision-making skills.
• Experience treating geriatric or Medicare-aged populations preferred.
• Ability to recognize changes in patient condition and coordinate interventions.
• Excellent communication and interdisciplinary collaboration skills.

Responsibilities

• Evaluate, diagnose, and treat acute illnesses and complex chronic conditions.
• Perform comprehensive assessments, physical examinations, and diagnostic evaluations.
• Manage patients with multiple comorbidities, including CHF, COPD, diabetes, and cardiovascular conditions.
• Recognize clinical deterioration and determine appropriate interventions or escalation of care.
• Order and interpret diagnostic testing, laboratory results, and imaging.
• Develop individualized treatment plans and manage medications.
• Coordinate post-hospitalization care to reduce readmissions and improve outcomes.
• Collaborate with physicians, nurses, specialists, and interdisciplinary teams.
• Educate patients and families regarding treatment plans and disease management.
• Maintain accurate documentation using electronic health records.
