Job summary
Houston Kidney Specialists Center (HKSC) is seeking an Acute Care Nurse Practitioner to join our nephrology team in the Houston area.
The role emphasizes collaboration with nephrologists, patient education, and care planning across inpatient and outpatient settings, with mentoring and specialized nephrology training available.
Bilingual Spanish/English candidates are preferred; AGACNP or FNP with Adult Gerontology certification is required and Texas NP license is needed.
Qualifications
- Master's or Doctoral degree in Nursing from an accredited program.
- Current Acute Care Nurse Practitioner certification (AGACNP) or Family Nurse Practitioner (FNP) with Adult Gerontology certification.
- Active Texas Nurse Practitioner license or eligibility to obtain.
- Minimum of one year of Nurse Practitioner experience.
- Strong clinical assessment, diagnostic, and care management skills.
- Excellent communication and collaboration abilities.
- Nephrology or dialysis experience preferred, but not required.
- Bilingual Spanish/English preferred.
Responsibilities
- Evaluate, diagnose, and manage patients with acute and chronic kidney disease.
- Collaborate closely with nephrologists and interdisciplinary care teams.
- Develop and implement treatment plans to improve patient outcomes.
- Provide patient education regarding kidney health, treatment options, and disease management.
- Document patient encounters accurately and efficiently within the EMR.
- Participate in care coordination across outpatient and inpatient settings.
- Manage a small subset of dialysis patients as part of the practice's comprehensive care model.
