• BA in social work required.

• Must be able to work independently and as part of a multi-disciplinary team.

• One year of previous acute care setting
• Able to provide independent transportation
• Current Licensure from State of Ohio Counselor and Social Worker Board

Responsibilities

• Resolve social or emotional problems that impede treatment of the patient’s medical condition.
• Assist physician, patient, and staff in understanding the social and emotional factors related to health problems.
• Work includes time split between office, travel within the Agency’s service area, and home visits; some out of county travel may be required.
