Novant Health is seeking a Clinical Pharmacist Generalist to join the acute care team at Presbyterian Medical Center in Charlotte, NC. This role focuses on delivering high-quality, patient-centered pharmaceutical care while collaborating with interdisciplinary teams to optimize patient outcomes.

The position emphasizes clinical excellence, teamwork, and building relationships with patients and their support systems within a fast-paced hospital setting.

Qualificações

• Bachelor's Degree in Pharmacy required; FPGEC certification required; PharmD preferred.
• Postgraduate year 1 residency or two years of hospital pharmacist experience; oncology experience preferred for oncology roles; required.
• ASHP PGY2 Infectious Disease residency or 2+ years in infectious diseases/antimicrobial stewardship required for Infectious Diseases roles.
• Current Pharmacy licensure in appropriate state or to be completed within 30 days of start date; Board certification or equivalent must be obtained within 2 years.

Responsabilidades

• Cross cover in adult acute care areas as needed.
• Facilitate pharmaceutical care services and drug utilization activities in assigned services; align with patient care needs and regulatory standards.
• Complete patient monitoring and review charts to identify or mitigate drug-related problems.
• Collaborate with multidisciplinary teams to provide patient-centered care.
• Provide evidence-based, patient-centered medication recommendations.
• Participate in the management of medical emergencies.
• Provide discharge medication review, reconciliation and counseling as appropriate.
• Maintain proficiency in central dispensing functions and operational programs.
• Oversee pharmacy operations and technician activities.
• Precept and mentor staff and students.
