Job summary

Mercyhealth Hospital and Medical Center - Harvard, IL seeks an Activity Assistant for the Long Term Care unit. This PRN role conducts activities assessments for new admissions and delivers a range of patient-centered activities in the evenings and weekends.

You will maintain a monthly activities calendar, collaborate with OT/PT/SLP and document participation in the flowsheet. A high school diploma and BLS within 90 days are required; EPIC EMR experience is helpful but not required.

Qualifications

• High school diploma or GED; or 1–3 months related experience.
• BLS certification within 90 days of hire.
• EPIC EMR experience helpful but not required.

Responsibilities

• Conducts activities assessments for all new admissions to the rehab unit.
• Provides activities including crafts, cooking, Wii, bingo and other options.
• Maintains a monthly calendar/schedule of activities.
• Seeks input and direction from rehab staff to provide patient-centered activities.
• Completes charting in flowsheet to document participation and tolerance to activity.
• Communicates with interdisciplinary rehab team.
• Most activities occur in evenings or on weekends.

## Activity Assistant - Long Term Care- PRNApply: Mercyhealth Hospital and Medical Center - Harvard, IL: PRN: Posted Today: 2026-02753**Overview:** Conducts activities assessments to determine patients' interests and desired activities**Schedule:** PRN**Shift:** Variable-As needed**ESSENTIAL DUTIES AND RESPONSIBILITIES*** Conducts activities assessments on all new admissions to the Comprehensive Inpatient Rehabilitation Unit.* Provides activities including but not limited to crafts, cooking, Wii, bingo, etc.* Maintains a monthly calendar/schedule of activities provided.* Seeks input and direction from OT/PT/SLP staff to provide patient centered activities.* Completes computer charting in flowsheet format to document participation and tolerance to activity.* Communicates with interdisciplinary rehab team including physiatrist.* Most activities are provided in the evenings or on weekends.**EDUCATION AND/OR EXPERIENCE**High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience**CERTIFICATION/LICENSURE**BLS certification within 90 days of hire**OTHER SKILLS AND ABILITIES** Problem Solving - Identifies and resolves problems in a timely manner, gathers information and reports as appropriate.
