West Haven Healthcare is seeking a Social Services Director to lead and manage the facility's social services team, ensuring resident emotional and social needs are met in accordance with federal, state, and local standards.
The role requires collaboration with Administration and other departments, development of policies, staff training, budget oversight, and maintaining compliance and resident family communication.
The primary purpose of your job position is to plan, develop, organize, implement, and direct Social Services staff in accordance with current and existing Federal, State, and Local standards, as well as our established policies and procedures to ensure that the emotional and social needs of the resident are met and maintained on an individual basis.
Plan, develop, organize, implement, and direct Activity staff in the overall Social Services function in the facility.
Meet with Administration and other facility management personnel in planning social services functions.
Develop and maintain a good working rapport with other departments, as well as outside community health, welfare, and social agencies to ensure that social service programs are properly maintained and meet the needs of the residents.
Develop and implement department policies and procedures.
Keep abreast of current Federal and State regulations and recommend necessary changes as needed.
Review departmental policies and procedures and recommend changes periodically.
Develop and manage a departmental budget and monitor expenditures.
Serve on various committees of the facility as appointed by the Administrator.
Develop policies and procedures for the identification of medically related social and emotional needs for the resident.
Develop training and in-service programs that can be used by social service staff members.
Participate in community planning related to the interests of the facility and residents.
Interview residents and family members in a private office setting to ensure social service needs are discussed and met, and maintain communications with resident family members.
Participate in Discharge planning, social care plans, and resident assessments in a private office setting.
Ensure transportation needs of the residents are met.
Provide financial information and assistance options to residents/resident family members.
Refer residents/resident family members to social service agencies as needed.
Provide direction to Social Services staff members when dealing with problematic situations.
Record and maintain social service records.
Compile and maintain a departmental schedule.
Maintain confidentiality of all pertinent patient care information housed in the Social Services office.
Review complaints and grievances and make recommendations to the Administrator.
Ensure that the Social Services department has the necessary equipment and supplies to operate effectively.
Perform charting and record keeping duties as necessary.
