Job summary

The Shores of Worthington is seeking an Activity Assistant to support engaging programs for residents in a skilled nursing setting.

Responsibilities include planning activity calendars, leading group and individual activities, and coordinating with staff to meet residents' care plans. The role requires a compassionate, energetic individual with flexibility for evenings, weekends, and holidays.

Qualifications

• High school diploma or GED required.
• Experience in activities or recreation role in healthcare preferred.
• Must be able to work flexible hours, including evenings, weekends and holidays.
• CPR/First Aid certification a plus.

Responsibilities

• Assist in developing and executing daily, weekly, and monthly activity calendars that meet the diverse needs and interests of residents.
• Lead group and one-on-one activities such as games, arts and crafts, music, exercise sessions, outings, and social events.
• Encourage resident participation and promote a positive and inclusive environment.
• Transport or escort residents to and from activities as needed, ensuring safety at all times.
• Help decorate and set up for events, holidays, and themed programs.
• Maintain attendance records, activity documentation, and progress notes as required by facility policy and regulations.
• Collaborate with nursing staff, volunteers, and families to support individual care plans and preferences.
• Assist with developing individualized activity plans based on residents' cognitive and physical abilities.
• Monitor residents during activities and report any changes in mood, behavior, or condition to appropriate staff.

• Ability to work independently and as part of a team.
• Ability to push wheelchairs and assist with resident mobility when necessary.
• Must be able to work flexible hours, including evenings, weekends, and holidays.
• CPR/First Aid certification a plus.
• Must pass background check and meet all facility/state health requirements.

Physical Requirements:

• Frequent standing, walking, bending, lifting (up to 25 lbs).
• Ability to assist with physical tasks during activities and resident care.
