Responsibilities

  • Encourage participation and assist residents to events.
  • Document participation and resident progress in Activity Records.
  • Visit residents individually to provide stimulation and orientation as needed.
  • Assist with welcoming new residents and notifying changes in care level.

POSITION SUMMARY

Work in conjunction with the Director of Resident Activities to plan, organize, direct, and conduct appropriate activities for residents that meet the mental, social, creative, and recreational needs of each.

ESSENTIAL FUNCTIONS
  • Research and select appropriate activities according to the interests, needs, and abilities of each resident to ensure maximum participation. (Maintain a balance of recreational activities, including physical, social, religious, diversional, creative arts and crafts, and occupational interest)
  • As needed or as instructed, interview and complete Activities Assessment Form upon admission, either with the resident and/or family members. Reassess periodically for changes to skills and abilities based on changes in condition of residents.
  • In conjunction with Director, plan, organize, direct, and conduct various activities daily, utilizing Activities Assessment Forms, Nursing Department feedback, and observation of residents’ skills, abilities, and interests.
  • Set up rooms for activities as appropriate.
  • Encourage participation in scheduled events to meet established resident goals.
  • Assist residents to and from designated areas for scheduled events.
  • Assist with planning, organizing, and overseeing specialized activity programs for selected groups or individuals as needed.
  • As needed or as instructed, document participation, abilities, and behavior in each resident’s Activity Record.
  • As needed or as instructed, visit with residents one‑on‑one, providing sensory, stimulation, and reality orientation as appropriate to resident needs.
  • Assist Director with welcoming each new resident to help ensure a comfortable transition.
  • Advise Director of any changes in level of care or resident’s condition. (Alert Charge Nurse to any changes that could signify changes in a resident’s condition)
  • Assist with creation of a monthly Activities Calendar.
  • Post copies of the Activities Calendar in each resident’s room, in Social Areas, and provide copies to Management Team, prepare copies to have available for family members, volunteers, visitors, and staff members, and post signs for special events, and assist with sending personal invitations when appropriate.
  • Assist in planning, organizing, and conducting special events.
  • Coordinate with Director of Resident Activities to ensure residents are recognized on birthdays, holidays, and other special recognition days as appropriate.
  • Create and decorate bulletin boards in public areas as requested.
  • Assist with decorations for holidays as appropriate.
  • Alert Charge Nurse, Maintenance Department, Director of Resident Activities, Environmental Services Department, or other appropriate staff of conditions that present a health or safety hazard to residents, staff, or visitors.
  • Report to Director of Resident Activities on the success and/or failure of activities conducted. Analyze reason(s) for failure and devise plans for changes based on analysis.
  • Conform to and perform duties in accordance with established facility policies as well as in compliance with Federal and State regulations.
MARGINAL FUNCTIONS
  • Coordinate with Dining Services Department for any dietary needs associated with scheduled activities.
  • Assist with feeding of resident meals as needed or as requested.
  • Attend in-service education and training as directed by facility policy and/or Director of Resident Activities.
  • Assist Director of Resident Activities and other Activities Department staff members with outings on the facility bus, including acting as Driver when needed.
  • Assist Volunteers as needed or as instructed.
  • Perform other related duties as assigned or required.
QUALIFICATIONS

Education/Certifications:

  • High School graduate or equivalent.
  • Current, valid Driver’s License.
  • Current CPR and Basic First Aid certification must be obtained within 90 days of hire.
  • CNA Certification required.
Experience:
  • Previous activities experience in a health‑care setting preferred.
  • Must be familiar with Microsoft Office software, including Word, Excel, and calendar programs.
Skills:
  • Ability to communicate clearly and effectively, both verbally and in writing, with residents/patients, staff, and family members.
  • Must exhibit patience and a positive attitude towards the elderly population and a desire to provide a service to the community.
  • Must be an intuitive, creative thinker.
PHYSICAL DEMANDS AND REQUIREMENTS
  • Ability to stand, walk, sit, stoop, bend, and reach for extended periods.
  • Ability to lift up to 20 pounds frequently (up to 66% of the time spent in performance of duties) and up to 35 pounds occasionally (up to 33% of time spent in performance of duties).
  • Manual dexterity that allows grasping, carrying, and holding equipment and supplies as needed to perform tasks.
  • Ability to see, hear, and verbally communicate with residents, visitors, and staff members during performance of duties.
