Job summary

Jefferson Healthcare provides skilled nursing care and is seeking an Activities Director to lead engaging programs for residents. The role emphasizes coordination with nursing and social services to tailor activities to individual needs and preferences.

The position requires strong organization, adherence to care plans, and commitment to resident rights and infection control. Compensation is based on experience and benefits are provided with a dedicated care-focused culture.

Responsibilities

• Plan, schedule and implement a program of individual and group activities based on resident's schedule.
• Complete the activity component of the resident assessment within 14 days of admission and review/update quarterly.
• Use RAPs to determine whether to proceed to care planning for activity-related concerns.
• Gather data and document required ancillary assessments including RAP summary documentation.
• Identify and document in the resident's care plan the activity interests, needs and concerns; review quarterly.
• Document all interactions with resident and/or family in assessment, care plan and progress notes as required by law.
• Participate as an active member of the interdisciplinary team and attend all resident care conferences.
• Plan and implement Reality and Orientation programs when appropriate.
• Plan and conduct Resident's Council meetings.
• Plan and implement evening and weekend functions as necessary.
• Attend in-service education programs related to Activity Program.
• Organize and schedule community events related to resident's interests.
• Supervise barber and beauty parlor activities as needed.
• Plan and schedule room visits and in-room activities for residents unable to leave rooms.
• Plan and implement indoor and outdoor activity programs.
• Maintain an activity attendance record for each resident.
• Assess needs and maintain supplies to implement activity programs.
• Observe infection control procedures.
• Follow Residents' Rights policies at all times.
• Adhere to facility policies & procedures and carry out disciplinary action for non-compliance as appropriate.
• Ensure adherence to Corporate Integrity Program and the Performance Expectations at all times.
• Perform other related duties as directed by the Administrator and/or person of authority.
