A leading healthcare facility in Walnut Creek is seeking an Activities Assistant to engage residents in meaningful activities. The role includes planning events, supporting communication between staff and families, and maintaining documentation. Ideal candidates will have a high school diploma and effective communication skills, with a preference for experience in long-term care settings.
Participate in planning and conducting individual, small, and large group activities. Assist in providing effective communication between employees at all levels, residents, their families, support personnel, government agencies/personnel, and the public to meet the needs and best interests of residents, the community, and the facility.
Participate in community planning related to the facility's interests and the needs of residents and their families. Assist in developing monthly activity calendars and maintaining attendance records. Support the Quality Assurance and Assessment Committee in creating and implementing plans to address quality deficiencies, as requested by the Activities Director.
Participate in discharge planning, develop and implement activity care plans, and conduct resident assessments. Assist in arranging transportation for residents, including outings, appointments, or discharges. Develop a monthly activity schedule that includes outings, group activities, and in-room activities for bed-bound or isolated residents. Encourage residents to participate in self-initiated activities such as hobbies, crafts, and reading, providing materials like Braille or audiobooks as needed.
Assist with documentation, including quarterly progress notes as assigned. Keep the Activity Department clean, organized, and secure.
This position has no supervisory responsibilities.
Requires standing and walking frequently, sitting occasionally, reaching, pushing, pulling, talking, hearing, tasting, smelling, and lifting up to 50 pounds regularly. Occasionally involves climbing, balancing, stooping, kneeling, crouching, or crawling. Visual requirements include close, distance, color, peripheral, and depth perception with adjustable focus.
