Job summary

Clinica Family Health & Wellness in Longmont, CO seeks an ACT Therapist (Licensed or Licensed Eligible) to deliver Assertive Community Treatment in an integrated outpatient setting. You’ll support adults with serious mental health challenges through brief psychotherapy and coordinated care.

Qualifications

• Masters in Psychology, Social Work, Counseling or related field plus 1+ year internship
• 1+ year experience in a clinical setting
• LCSW/LPC or full license within 24 months from hire
• Experience providing clinical assessments and brief intervention
• Mastery of evidence-based psychotherapy models
• Experience in an integrated health environment

Responsibilities

• Provide brief episodic psychotherapy addressing mental health issues and physical health concerns
• Manage a small caseload of high-acuity clients needing psychotherapy and complex case management
• Collaborate with a multidisciplinary team and external providers to support recovery
• Coordinate care to support clients’ recovery in an ACT outpatient setting

Job Summary:

Assertive Community Treatment (ACT) is a specialized outpatient program that helps adults experiencing serious and long-lasting mental health challenges . The ACT team meets with most clients several times a week, giving support when and where it is needed. Seeing clients more regularly helps us play a bigger part in their healing and recovery journey .

What we need for this job:

• Masters in Psychology, Social Work, Counseling or related field, plus 1+ year internship, and one (1) or more years experience in a clinical setting
• Or Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC)
• Or full licensed acquired within twenty-four (24) months from hire
• Experience providing clinical assessments and brief intervention
• Mastery of evidence-based psychotherapy models and practice
• Experience working in an integrated health environment
