Job summary

EMPACT-SPC, part of La Frontera Arizona, seeks an ACT Team Specialist to join our community-based clinical team serving the Phoenix Metro area. You will participate in assessments, develop ISP, and coordinate care across families and providers, ensuring services align with recovery goals.

Shift: Wednesday-Saturday. We offer stability, a collaborative environment, and comprehensive benefits for full-time staff, including PTO, holidays, medical, dental, vision, and retirement plans.

Qualifications

• Bachelor's degree in a behavioral health-related field preferred.
• Experience in community behavioral health with adults with serious mental illness required.
• ACT Team experience preferred.
• One year experience working with SMI preferred.
• Bi-lingual (Spanish) is a plus.
• Available to work; Days, Weekends, and On-call.
• You must have a Fingerprint Clearance card or be eligible to obtain one.
• Valid AZ driver's license with no major infractions in the past three years.
• Must be 21 years of age or older at the time of employment.

Responsibilities

• Participate in assessment and service plan development and implement services to consumers per ISP.
• Communicate and document consumer progress toward recovery and coordinate care with providers.
• Engage, educate, and coordinate with consumer, family, behavioral health, medical and dental providers.
